  • Overview

    Linkage Navigator needed to implement successful community social mobilisation strategy strongly supported by relevant and successful linkages and referrals (care & treatment and VMMC) within PEPFAR and DoH community HIV programs in  KZN Province (uMgungundlovu, uThukela, uMkhanyakude, uGu, King Cetshwayo/uThungulu, eThekwini) and Western Cape (City of Cape Town)  Linkage Navigator reports to the Demand Generation Officer and supervises Social Mobilisers.

     

     

     

     

     

     

    Responsibilities

    • Ensure VMMC clients are successfully linked to other health services
      • Identify and work with local community HIV program implementing partners to successfully link and refer VMMC and HTS clients accordingly
      • Liaise with facility HTS counsellors and other professionals to link eligible men to VMMC services
      • Liaise with VMMC teams to successfully link VMMC clients to HTS local partners
      • Place a dedicated social mobilizer in a health facility offering both HTS and VMMC services to facilitate the physical linkage of eligible men within that facility
      • Disseminate all relevant linkage and referral M&E tools to staff and clients
      • Attach VMMC social mobilizers to various local community HTS partners conducting community HTS activities to facilitate active and successful VMMC linkages
      • Invite community HTS implementing partners to VMMC campaigns and camps to facilitate an active same time linkages to HTS services
      • Provide support supervision for the integration of VMMC into Community health activitie
    • Supporting VMMC demand generation strategy    
      • Plan and coordinate VMMC mobilization activities in allocated area
      • Work with relevant stakeholders to organise successful VMMC activities e.g. camps, campaigns, outreaches and mobile services.
      • Provide social mobilizers with relevant IEC materials, stationary and other resources to facilitate and improve performance
      • Be exemplary to your social mobilizers by also conducting mobilization of VMMC clients
      • Work with social mobilizers to identify innovative mobilization strategies that will enable adult men to seek VMMC services
      • Participate in meetings and planning with local social mobilization partners  
      • Assist counsellors and mobilizers of local CBOs/FBOs partnering with Jhpiego with VMMC mobilisation strategies
      • Ensure and facilitate that Jhpiego social mobilizers partner and participate to activities of local municipality, traditional, religious and youth sectors
    • Support and management of social mobilizers

      • Deploy social mobilizers to relevant areas, zones and facilities to ensure proper coverage of your allocated geographic area including men infested areas e.g. work places, hostels, tertiary

      • Participation in local demand creation meetings and workshops with mobilizers
      • Monitor and assist social mobilizers to achieve monthly targets as planned
      • Provide social mobilizers with relevant M&E material e.g. monthly report, referral and claim forms/registers
      • Facilitate completion of monthly forms by mobilizers and complete monthly performance report and submit accordingly
      • Work with clinical team leader/coordinator to plan outreaches, camps and mobile services informed by number of clients mobilised.
      • Support mobilizers with printed materials to facilitate community engagements e.g. consent forms, VMMC basic facts information, pamphlets, card contacts etc.
      • Provide social mobilizers with relevant working materials like uniforms, caps, umbrellas etc.
      • Organise transportation of social mobilizers where necessary to access strategic areas like taxi ranks, malls, community gatherings and attending to presentation appointments
      • Perform any other duties as assigned by the VMMC demand creation officer

     

    Required Qualifications

    • Matric
    • Certificate in HIV & AIDS/health education, management/administration
    • Certificate in health/social related qualification
    • 2 years community experience in education and training
    • Organising community activities e.g. campaigns, activations
    • Ability to work independently, under pressure and efficiently
    • Proficiency in local languages                                                           
    • Sound interpersonal skills and must be able to engage with MMC stakeholders, community, traditional and political leaders
    • Good communication skills (verbal and written)
    • Creative and Innovative
    • Leadership
    • Valid driver’s license
